My girlfriend has been begging for one of these so I thought I would post it and see what comments we get back.
This, well, rather adorable Crocheted Notebook Sleeve is handmade by Etsy seller SpecialMeat. For just $20, SpecialMeat will hand-make you a notebook sleeve per your specifications.
You pick the dimentions, color, embellishments and everything else. Not too bad for a $20 spot.
